Vitamin D and Bone Health: Calcium, Testing and Who May Need Supplements

Vitamin D helps the gut absorb calcium, while calcium provides much of the mineral structure of bones and teeth. Both are essential, but stronger bones do not come from taking the largest possible supplement dose.

Bone health also depends on exercise, nutrition, hormones, falls and medical conditions. People with osteoporosis may need treatment that supplements cannot replace.

Key point: use supplements to meet an identified need or dietary shortfall - not to chase a universal vitamin D blood target.

How vitamin D and calcium work together

Bone is living tissue that is continually broken down and rebuilt. Vitamin D supports active calcium absorption and helps maintain calcium balance. Severe vitamin D deficiency can cause osteomalacia, in which adult bones become poorly mineralised and painful or weak. [2,3]

A routine blood calcium result does not show whether someone eats enough calcium or has strong bones. The body tightly controls blood calcium, partly by drawing it from the skeleton when necessary. Bone strength is assessed through the wider clinical picture and, when indicated, bone-density testing.

Do supplements prevent osteoporosis or fractures?

The answer depends on who is taking them and why. In generally healthy adults without an established indication, routine high-dose vitamin D has not consistently prevented fractures. The Endocrine Society advises that healthy adults under 75 are unlikely to benefit from taking more than the usual recommended intake solely for disease prevention. [1]

For people with osteoporosis, a fragility fracture, low intake, malabsorption or confirmed deficiency, calcium and vitamin D may form part of care. They do not replace exercise, falls prevention or osteoporosis medicine. Dietary calcium is generally preferred, with supplements targeted to gaps. [4]

Who may need individual assessment?

A clinician or dietitian may review vitamin D, calcium intake or both for:

  • Confirmed vitamin D deficiency, osteomalacia, osteoporosis or a fragility fracture
  • Older adults who are housebound, frail or in residential care
  • Very limited calcium intake, including some dairy-free diets
  • Coeliac disease, inflammatory bowel disease, bariatric surgery or other malabsorption
  • Chronic kidney or liver disease, parathyroid disorders or recurrent kidney stones
  • Medicines that affect bone or vitamin D, including long-term glucocorticoids
  • Osteoporosis treatment that requires adequate calcium and vitamin D

Limited sun exposure, covering most of the skin and darker skin pigmentation can increase the likelihood of low vitamin D, but none diagnoses deficiency. Pregnancy, breastfeeding and childhood use should follow local guidance; adult doses should not be copied for children.

Which tests may be useful?

Vitamin D blood test

Vitamin D status is usually assessed with serum 25-hydroxyvitamin D, written as 25(OH)D. Routine screening is not recommended for every healthy adult without an established reason to test. Interpretation depends on the clinical context, laboratory method and local guidance; there is no single universal target for disease prevention. [1,2]

Bone-density test

A DXA scan measures bone mineral density; it is different from a vitamin D or blood calcium test. Screening recommendations vary by country, age, sex and fracture risk. Previous low-trauma fracture, long-term steroid use, early menopause and significant height loss are reasons to discuss assessment. [5]

Calcium: food first where practical

Useful sources include milk, yoghurt, cheese, calcium-fortified plant drinks, calcium-set tofu, small fish eaten with bones, beans and some leafy vegetables. Check labels because fortification varies. National targets differ; count food before deciding how much, if any, is missing. [3]

Choosing a supplement safely

Vitamin D

Vitamin D3 (cholecalciferol) and D2 (ergocalciferol) can both raise vitamin D levels. Labels may use micrograms or international units: 1 microgram equals 40 IU. Check multivitamins and combination products so the same nutrient is not counted twice. Avoid repeated large loading or intermittent megadoses unless a clinician has prescribed and monitored them. Calcium

The important label figure is elemental calcium, not the total weight of the calcium compound. Calcium carbonate is usually taken with food; calcium citrate can be taken with or without food and may suit people with reduced stomach acid. Absorption is better from smaller amounts - around 500 mg or less at one time - than from one large dose. [3]

Side effects, interactions and excessive intake

Calcium supplements can cause constipation, bloating or nausea, and excessive supplemental calcium may increase kidney-stone risk. Too much vitamin D can cause high blood calcium, leading to nausea, vomiting, weakness, confusion, unusual thirst, frequent urination and kidney problems. [2,3]

Calcium can reduce absorption of levothyroxine, iron, bisphosphonates, dolutegravir and some antibiotics. Thiazide diuretics and other medicines can alter safety. Follow the product-specific interval recommended by a pharmacist or prescriber.

Seek urgent advice after suspected overdose, accidental child ingestion, severe vomiting, confusion, dehydration or an abnormal heartbeat. Before supplementing, seek individual advice if you have kidney disease, stones, high calcium, hyperparathyroidism, sarcoidosis or another granulomatous condition.

A practical decision framework

Estimate calcium and vitamin D from food, fortified products and every supplement you already use.

Review fracture risk, medical conditions, pregnancy status and medicines with an appropriate professional.

Test only when there is a clinical reason, then use the result and local guidance to plan treatment and follow-up.
